Air Trapped the Most Common Cause (and the Easiest Fix)
If the top of the radiator is cold but the bottom is warm, air has collected at the top and is pushing the hot water down and out before it can heat the whole panel. This happens naturally over time — every time your system is drained, topped up, or a new radiator is added anywhere in the house, a bit of air gets introduced.
If you have several radiators to bleed, work from the radiator closest to the boiler outward, and always recheck the pressure gauge once you're done.
Sludge and Debris: Why the Bottom Stays Cold
